As a new media industry and LA native I have not been surprised but heavily disappointed at the lack of traction in solving some of the largest industry issues using blockchain. Why is the industry continuously not learning its previous hard lessons and behind the ball on what is the 2nd wave of the revolution that shook the industry in the first place? And how do we change it?

Which issues? What were the previous lessons not learned, and why does the industry need to solve them, rather than others?
There are in fact companies using blockchain to solve complex rights management issues. For example, Mediachain, recently acquired by Spotify, solves attribution using blockchain. I expect there are many others.
One could take the Digital Object Identifier System, which has a very clearly defined specification for digital object rights management, and use blockchain to build a solution.
