Hip Retroversion - Definition, Science, Diagnosis

SJD may play a role in acetabular arrangement, leading towards more hip impingement with the retroverted femur. So having the SI joint assessed and treated, may be a conservative option in addition to other treatments.

Labral Tear

Passive conservative management includes anti-inflammatory medications and active conservative management includes physiotherapy...emphasized on hip and lumbopelvic stabilization, correction of hip muscle imbalance, biomechanical control, and sport-specific functional progression....

Encouraging results from four patients, each with an acetabular labral tear confirmed by MRI, which show that meaningful improvement can be achieved with nonsurgical intervention! Hopefully, surgery may thereby be averted.

Additional Non-Invasive Therapies

Studies shown that hydrotherapy has a significant impact on pain reduction, improved strength and improvement in quality of life1,2,3,4.Even though there are not article related to pain relieve due to femoral retroversion, but I think it is a good therapy to give it a try.

Have the patient tried chiropractic manipulation? There are studies that shown that various hip condition (hip osteoarthritis, femoraacetabular impingement) have improve with chiropractic management.

Surgical Option Risk and Reward

HO would be another risk, in the case we're discussing, as it is usually triggered from surgery in young adults (her age) and can be a severe limiter on joint range and impingement. Another reason to try and avoid surgical intervention.

Since no amount of conservative therapy will alter the aberrant anatomy of the joint, and the predisposition impingement and crushing of the labrum will thereby persist, surgical correction may ultimately be necessary, particularly if patient intends to continue her rigorous athletic participation.

Cortisone Injections

Cortisone injections - the most likelky type of cortisone therapy would be an ultradsound guided injection into the bursa. This is a very commonly used therapy that often gives 3 to 9 month decrease in pain from inflamed or compromised bursa.

there is always a chance to puncture the blood vessel or injecting the cortisone at the torn labral/ligament or other undesirable site impeding healing process. Thus, it is better to use US guided injection technique rather than blind puncture.

1) Functional Movement Therapy 2) Chiropractic care 3) Water therapy:

Physical therapy to strengthen the supporting musculature Rest, ice, compression, and elevation (RICE) to limit acute pain

1) Adjust sports activity...taking a break...to get pain reduction...allow other ... therapies time.., 2) Physiotherapy to reduce pain, improve muscle strength and functional mobility. 3) Anti-inflammatory medication...(non-opioid medications) that offer relief during periods of severe discomfort

Acupuncture

“Our study found evidence for the effectiveness of acupuncture for 117 conditions, with stronger evidence for acupuncture’s effectiveness for some conditions than others. The quality and quantity of research into acupuncture’s effectiveness is increasing.”

Studies shown that the mechanisms of action of acupuncture point stimulation is related to release of endogenous opioid peptides in the central nervous system that mediates the analgesic effect1,3,4.

These acupuncture points are located at sites that have a high density of neurovascular structures and are generally between or at the edges of muscle groups1 which are less painful than random needle sticks into a muscle group2

New Recommendations

Could platelet rich plasma regeneration therapy (PRP) could aid to avoid surgery?

There are various techniques of electro stimulation for analgesia and recovery used by physiotherapist specialized in sport science1. Studies shown that patient received the transcutaneous electrical nerve stimulation (TCES) has significant improvement in pain modulation and functional quality2.

Studies shown that TU has numerous application in sport science especially for speeding up soft tissue injury and bone healing. It works on all repair phases where it stimulate tissue repair and wound healing1

Pain Management

Many are using Cbd for pain, I use an oil, that now comes in softgels, called Copiabia. It's without the thc and I have many great testimonials. I am all for resolving the issue, rather than masking the symptom which Cortisone does, plus there are no side effects with Copiabia.

I like the idea by using Cbd and acupuncture. In fact there is a study that state that acupuncture works in synergy with morphine there by reducing its dosage for pain relieve. I think that it could also couple with cbd providing an add on effect.

Chiropractic Care

Basically, the hip is where the "backbone is connected to the leg bone" :-) If the pelvis is rotated too far back, giving a deep spinal curve, the hip joint is rotated rearwards, as is the femur head joint

Compression & RICE

studies found compression garments could improve venous compression on the limbs1 as ...and therefore, enhance recovery...also reduces the intramuscular space available for swelling promotes alignment of muscle fibers, blunting inflammatory response, reducing muscle soreness.
