All of the things you list are important enablers - but the central things that will enable effective patient engagement all center around knowledge and ownership of personal data. Each patient should be in almost total control of access to their data. This, of course, is very difficult in the current environment where every entity except the patient thinks it owns the data and fights for a controlling interest in that data. Centralizing ownership and accessibility with the patient gives them the basis to increase their knowledge and influence over the care they are given and enables them to share the data with relevant care givers (family, physicians, etc.). This is liberating and gives the patient control - which in turn should lead to greater engagement.
